About this Program:
The ABC’s of Employment Forms is designed to review and examine forms and records essential to the effective management of employee issues. Appropriate forms help to protect employers against costly mistakes, avoid litigation and ensure compliance with state and federal laws.
This program will review some of the more commonly used (and unused!) employment forms which should be in your arsenal, including:
ª Hiring Documents, Applications, checklists of legal and illegal inquiries, consents to background checks, substance abuse testing forms, confidentiality agreements and non-compete covenants.
ª Personnel File Documents– Immigration Forms, Tax Forms, Handbooks and Acknowledgments, Evaluations, EEO Reports, Workers’ Compensation and Unemployment compensation forms.
ª Leave of Absence documents, including FMLA requests, return to work certifications.
ª Restrictive Covenants (confidentiality, non-competition and non-solicitation agreements).

Your Instructor:
Tom Dixon, Esq.
Mr. Dixon, of Eastman and Smith, practices throughout Ohio in the area of management labor and employment law. His practice covers the range of employment issues, including counseling on employment policies and handbooks, training of supervisors and managers, representation in administrative proceedings, defending discrimination claims, and providing guidance on the Americans With Disabilities Act, the Family and Medical Leave Act, drug testing, sexual harassment, wage‑hour matters, and a variety of other issues. He also provides assistance on union organizational campaigns, union avoidance, negotiation and administration of labor contracts, labor arbitration, wrongful discharge, employment‑at‑will litigation and civil rights litigation. |
